IMPORTANT! When you have successfully made an appointment on the scheduler, you will receive a confirmation e-mail. If you do not get a confirmation, you do not have an appointment. When you get the confirmation, please double-check the date and time to make sure it's what you wanted & that you've entered it correctly in whatever other calendar you use. If you're having problems scheduling, please call me (303) 545-5792 x 103.

To cancel an appointment more than 24 hours in advance:
Access the Review Appointments screen in 2 ways:
1) click the link in the confirmation email, or
2) go to the client scheduler and select Review Appointments on top right.

Provide the appointment confirmation number given in the email
Enter the email address provided when the appointment was booked.
(The Review Appointments screen lets the client review details of any existing appointments.)
Select an appointment and click the Cancel Appointment button to cancel the appointment.

Fees & Policies

Fees:
Adults $95.00
Children (through middle school) $55.00

Payment is expected at time of service. I accepted Visa, Mastercard, checks and cash. If your insurance allows you to see a physical therapist who is not a preferred provider or you have a flex plan that will reimburse you , I can provide you with a “superbill” that you can submit for reimbursement.

Cancellation and late policies
I respect your time and usually begin appointments promptly. In those rare occasions that I am more than 20 minutes late for your appointment and need to shorten your appointment because of it, your fee will be prorated, or you will have the option of rescheduling for another time. If you are going to be late, please call me if you can. I may be able to do everything that needs to be done in the remaining time, but I may need to run late, into the next patient’s scheduled time. Please respect their time and make every effort arrive promptly for your appointment. If you are going to be more than 20 minutes late, you may be charged half-price for the office visit and we will have to reschedule.

If you need to cancel your appointment, please give 24 hours notice. Less than 24 hours notice or not showing up for your appointment may result in you being charged for the office visit.
